Koklear İmplantasyonun Bireylerin Yaşam Kalitelerine ve Maliyet-Değer Analizlerine İlişkin Araştırma

Özet

Amaç: Bu çalışmanın amacı işitme kaybı bulunan bireylerde yapılan tek taraflı ve çift taraflı koklear implant işlemlerinin yaşam kalitelerine etkilerini ve maliyet-değer analizlerinin karşılaştırılmasına ilişkin araştırma yapmaktır.

Yöntem: Son 20 yılda PubMed’de yayınlanan araştırmalardan, “tek taraflı koklear implant”, çift taraflı koklear implant” ve “maliyet-değer analizi” anahtar kelimeleriyle taranarak elde edilen makaleler değerlendirilerek, koklear implant işlemlerinin bireylerin yaşam kalitelerine etkileri ile maliyet-değer analizleri incelenmiştir.

Bulgular: Sağlık Kullanım İndeksi-3 (HUI3) genel olarak koklear implant işlemlerinin maliyet-değer çalışmaları için Sağlıkla İlişkili Yaşam Kalitesi (HRQoL)'nin en ihtiyatlı ve tutarlı ölçüsü olarak kabul edilmiştir. Daha erken yaşta implant işlemi uygulanan çocukların yaşam kalite puanlarının daha yüksek olduğu görülmüştür. Tek veya çift taraflı koklear implant’ın hangisinin yapılacağına karar aşamasında, çift taraflı koklear implant’ın yaşam kalitesi kazancının daha yüksek, artırımlı maliyet-değer oranın daha düşük olduğundan dolayı çift taraflı koklear implant’ın seçilmesinin daha uygun olacağı düşünülmektedir.

Sonuç: Tek veya çift taraflı koklear implantasyon öncesi ve sonrasında yaşam kalitesinde istatistiksel olarak anlamlı artışlar bulunurken, son derece uygun maliyet-değer oranları bulunmaktadır. Türkiye’de yapılan koklear implant işlemlerinin maliyet-değer analizi çalışmalarının yapılarak gelişmiş ülkeler ile kıyaslamasının yapılması önerilmektedir.
