Serebral Palsi’li Adolesanlarda Solunum Egzersizinin Ağrı Düzeyi, Denge, Yürüyüş ve Yaşam Kalitesi Üzerine Etkisi: Tek Kör, Randomize Kontrollü Çalışma

Özet

Amaç: Bu çalışma, adolesan serebral palsili bireylerde konvansiyonel fizyoterapi programına ek olarak uygulanan solunum egzersizinin, ağrı düzeyi, fonksiyonel kapasite, yaşam kalitesi üzerine etkisini incelemek amacıyla planlandı.

Gereç ve Yöntem: Bu randomize kontrollü çalışmaya Kaba Motor Fonksiyonel Sınıflandırma Sistemine göre seviye 1-2 olan 27 adolesan serebral palsili (12-17 yaş arası) birey katıldı. Katılımcılar randomize bir şekilde kontrol grubu (n:13, ortalama yaş: 13.8±1.7 yıl ve çalışma grubu (n:14, ortalama yaş: 13.5±1.8 yıl) olmak üzere ikiye ayrıldı. Her iki grup da 8 hafta boyunca nörogelişimsel fizyoterapi programına katıldı. Buna ek olarak çalışma grubuna 8 hafta, haftada her gün, 15 dk diyafragmatik solunum egzersizinden oluşan solunum egzersizi eğitimi uygulandı. 8 haftalık fizyoterapi programı öncesi ve sonrası her iki grubun ağrı seviyesi Vizuel Analog Skala (VAS) ile, denge Zamanlı Kalk ve Yürü Testi (ZKYT) ve Pediatrik Berg Denge Ölçeği (PBBT) ile, yürüyüş Gillette Fonksiyonel Yürüme Değerlendirme Anketi (GFYDA-GFYDA22) ile, yaşam kalitesi Çocuklar İçin Yaşam Kalitesi Ölçeği (ÇİYKÖ) ile değerlendirildi.

Bulgular: Her iki grupta da tedavi sonrası tedavi öncesine göre; denge, yürüme performansı, yaşam kalitesi parametrelerinde artış görüldü (p<0.05). Çalışma grubunda solunum egzersizi sonrası ağrı düzeyi 6.8±0.7’ den 5.1±0.5’ e (p=0.0001), kontrol grubunda 6.5±0.1’ den 6.3±1.2’e (p=0.083) değişti. Fizyoterapi programı sonrası çalışma grubunda kontrol grubuna göre ağrı düzeyinde istatistiksel olarak anlamlı iyileşme görülürken (p=0.005) diğer parametrelerde iki grup arasında anlamlı bir fark bulunmadı (p>0.05).

Sonuç: Solunum egzersiziyle birlikte uygulanan fizyoterapi programının adolesan serebral palsili bireylerde ağrıyı azaltmada etkili olduğu ve ağrıyı azaltmak için rehabilitasyon programlarına eklenebileceğini düşünüyoruz
