In the management of chronic disease, only the traditional treatment approach is not sufficient, self-management strategies in which the active participation of the patients is ensured play a key role in the management of chronic diseases. Self-management; It is the ability of an individual with a chronic disease to cope with the symptoms, complications, and physical and psychosocial consequences of treatment, and to integrate behavioral changes that need to be made to adapt to the disease. Aim; It is the ability of individuals to determine their needs and develop and implement multidimensional solution strategies in order to solve the problems they encounter in daily life with the right approach. Roles and responsibilities of nurses in self-management support; It includes gaining knowledge and technical skills for symptom recognition and self-monitoring of the individual and family in chronic disease management, supporting in the decision-making process, activating social and social supports, developing specific strategies and problem solving skills to set goals and create action plans to provide behavior change. . In order for self-management support programs to be effective and efficient, they should be individualized with frequent contact with health professionals for at least 6 months, teaching-learning methods should be maintained with a collaborative team approach using the latest technology included in a sustainable program strategy, and nurses, patients and families should be supported. Should be supported by training how to personalize the teaching time and content for self-management.
