Abstract

Diabetes Mellitus is at the forefront of chronic noncommunicable diseases resulting from defects in both insulin secretion and action, which represent a major challenge for global health. Diabetes mellitus is one of the diseases that burden both public health and health care costs worldwide. Practices and approaches to support and maintain diabetes self-management are vital. Mobile health applications are used in fields such as education, training, support in diagnosis and diagnosis, patient monitoring, control of epidemic diseases, storage of health data, mobile learning, payment, smart payment systems. Diabetes self-management education enables patients to participate in care by increasing their knowledge and skills. The most important role of diabetes educators is to ensure that diabetes patients have the most up-to-date information. Diabetes self-management education and support provided by telehealth for older adults with diabetes after hospital discharge has been observed to be feasible for telemedicine intervention for hospitalized older adults with T2DM.
