Abstract

Background: Providing adequate, continuous and quality health services in all countries is among the main objectives. Adequate, continuous and quality health care delivery requires a lot of resource consumption; This situation necessitates the effective and efficient use of resources.

Aim: The aim of this study; to evaluate the bed utilization efficiency of the intensive care units of a tertiary education and research hospital with 1270 beds in the A1 service role of the Ministry of Health.

Method: In the study, the Pabon Lasso model was used to determine the intensive care bed utilization performance of the hospital between 2019 and 2022. Within the scope of the research, a total of 8 intensive care services with a bed capacity of 7 or more were included. Intensive care units evaluated in this context, Anesthesia and Reanimation Intensive Care (ARYB), Surgical Intensive Care (STI), Pediatric Intensive Care (BID), General Intensive Care (ID), Coronary (Cardiology) Intensive Care (CIB), CVC Intensive Care (CID) KVCYB), Neurology Intensive Care (NYB) and Neonatal Intensive Care (NICU). Research data were obtained from the Basic Health Statistics Module of the relevant hospital. In the model, operational indicators such as bed occupancy rate (HCR), bed turnover rate (YFR) and average days of stay (OKG) were used. Excel program was used.

Results: According to the Pabon Lasso model applied in the light of the data obtained from the research data, in the 2nd Region, where KVCYB, low HDL, high HDH and short OKG were observed consistently in all years from the intensive care services included in the study; On the other hand, in the 3rd Region, where ARYB has high LCR, high YDH and short OKG; It was determined that the CGU is located in the 4th Region, which represents high LCR, low HDF and long OCG. It has been determined that while YYB was in the 2nd Region in 2019, 2021 and 2022, it moved to the 4th Region, which is a relatively inefficient region, in 2020. It was determined that the BID was in Region 4 in 2019, 2020 and 2022, and in Region 1 in 2021. It is seen that GYB is in Region 4 in 2019 and 2022, while in Region 1 in 2020 and 2021. It was concluded that while STI was a productive region in 2019, it was inefficient in 2020 and moved to the 4th Region in 2021 and 2022. While the KYB was efficient in 2019 and 2021, it is seen to be located in the 2nd Region, which is an inefficient region in terms of LDO in 2020 and 2022.

Conclusion: Since the Pabon Lasso Model graphically shows the low-performing clinics regarding the use of beds in hospitals, it provides important conveniences in terms of allowing hospital managers to make improvements in inefficient areas. According to the results of the research, making the necessary arrangements in the services located in the inefficient region in the intensive care services will also allow the resources to be used at the optimum level. It is recommended that hospital management develop strategies in this direction, taking into account the results of the study.
